GOOD SHEPHERD LUTHERAN SERVICES
NOTICE OF PRIVACY PRACTICES
POLICY AND PROCEDURES

PURPOSE:  To describe Good Shepherd Lutheran Services’ efforts to comply with the privacy rules’ requirements related to the Notice of Privacy Practices.
POLICY:  Good Shepherd Lutheran Services provides and uses its Notice of Privacy Practices in accordance with Privacy Rule Requirements.
PROCEDURE:
1.
The Notice of Privacy Practices is written in plain language and its content is in accordance with Privacy Rule requirements.
2.
The Notice of Privacy Practices describes Good Shepherd Lutheran Services’ uses and disclosures of Protected Health Information (PHI) and residents’ rights with respect to their health information.
3.
Residents are provided with a copy of Good Shepherd Lutheran Services’ Notice of Privacy Practices as soon as practicable upon admission.
4.
A good faith effort is made to obtain written acknowledgement that the resident has received a copy of the Notice of Privacy Practices.
5.
Workforce members receive training related to Good Shepherd Lutheran Services’ privacy practices as detailed in the Notice of Privacy Practices.
6.
The Notice of Privacy Practices is revised whenever there are changes to federal or state privacy laws and regulations or to Good Shepherd Lutheran Services’ privacy practices.  Revisions to the notice will not be implemented prior to the effective date of the revised notice and distribution of the revised notice to workforce members, residents and business associates.
7.
A copy of the Notice of Privacy Practices is retained for six years from the date it was last in effect.
